I can confirm that the adaptor that you have received (Friwo) is the correct adaptor which ships with as part of the ADAS1000 evaluation kit. This adaptor should provide the +5V required by the ADAS1000 evaluation board. It should not encourage damage to any of the board/components, so it seems something is certainly amiss with what you have received.
What Padraic was trying to figure out (picture above) was whether the little connector at the end of the adaptor was orientated correctly or not (this connector is fitted as part of the board build, it might be that for this particular adaptor, the orientation was inadvertently reversed). The connector should be inserted such that GND is on the outer side of the connector with +5V on the inner side of the connector. You can easily measure this with a DVM (with adaptor plugged in and connector not connected to anything). If you can measure this and let us know that'll help us understand whether this adaptor was made correctly/otherwise (and ensure that it's rectified).
Incidentally, if you look closely at the markings on the connector end (picture above), you should see an arrow and a "+" and a "-". Can you tell what aligns with the arrow? (or if there is something different on the model you have?)?

We will of course support replacement of the board (as it may now be damaged), so if you can return the entire kit (board/adaptor) through the source (disti) that you received it, they will provide a replacement.
